Während der isolierte thorakale und der isolierte abdominale Zugang grundsätzlich verschieden sind, haben alle thorakoabdominalen Zugänge Gemeinsamkeiten. Der Unterschied ergibt sich aus der Höhe des verwendeten Interkostalraumes, der Art der medialen viszeralen Rotation sowie dem Ausmaß der Zwerchfelldurchtrennung. Die Qualität des Zugangs kann in der Folge über den Operationsfortgang mitentscheiden. Zu berücksichtigen ist die Traumatisierung durch den Zugangsweg bei Operationen an der thorakoabdominalen Aorta, der in Abhängigkeit von der aortalen Pathologie und dem Gesamtzustand des Patienten gewählt werden sollte.
Summary
Surgery of the descending thoracic and thoracoabdominal aorta necessitates the thorough selection of the approach due to the specific aortic pathology. Even though, the selection seems to be simple, it may be difficult when the lesion seems limited and/or when the comorbidities of the patient leads to a compromise between achieving the ideal surgical exposure and limiting the trauma needed to reach it. The aim of this chapter is to describe the thoracic, abdominal, and thoracoabdominal approaches to the descending thoracic and thoracoabdominal aorta.
