La mise en place du systeme d'hemovigilance en France, avec la notification obligatoire des incidents transfusionnels, a permis d'enregistrer 185 incidents lies a la contamination bacterienne des produits sanguins labiles, 18 (10 %) ont entraine le deces du malade transfuse, dans 26 % des cas le pronostic vital du malade etait en jeu et dans 64 % des cas, la symptomatologie a ete moderee. Grace a ces observations, l'importance du risque a ete reevaluee. Il est estime aujourd'hui a 1 ITCB pour 135 000 PSL distribues. L'hemovigilance a permis d'attirer l'attention sur les incidents mineurs, dont l'expression clinique souvent moderee est une source de meconnaissance, avec le risque d'accident transfusionnel en chaine et de perte d'informations qui s'en suit. L'analyse des dossiers a permis de mettre en evidence l'implication des cocci Gram positif dans les accidents lies aux concetres de globules rouges. Les observations du reseau national d'hemovigilance confirment les difficultes de l'enquete etiologique, liees, notamment aux difficultes de l'expertise bacteriologique, mais ont permis d'attirer l'attention sur une eventuelle nouvelle complication immediate de la transfusion, que serait l'atteinte aigue de l'immunite non specifique. Sur les bases des observations du reseau d'hemovigilance, ont ete developpes un systeme specifique de surveillance et un programme oriente de recherche. Des mesures de prevention ont ete developpees a toutes les etapes de la chaine transfussionnelle et des recommandations ont ete adressees aux utilisateurs de produits sanguins pour favoriser la declaration et la prise en charge de ce type de complications.
Through the hemovigilance system implemented in France in 1994, 185 cases of transfusion reactions due to the bacterial contamination (TRBC) of blood components (BC) have been recorded over 5 years, thus confirming the frequency and severity of bacterial risks in transfusion medicine. Activation of the hemovigilance network and awareness raising among the virus actors involved (clinicians, nurses...) have made it possible to both prevent possible related TRBCs (due to the transfusion of a BC derived from the same donation) and improve case reporting. The first results obtained through the alert system thus developed have led the Agence Francaise du Sang to assess and better understand, in a joint study, transfusion-related risks as well as determine factors likely to prevent them. Continuous analysis, in real time, of the reported cases have improved prevention strategies at every stage of the transfusion chain, from blood collection to blood component transfusion.
